- Mobile – we come to you at the convenience of your home or preferred place of care and treatment for one on one sessions.
- Physical Therapy session (1 hour, including initial evaluation); specializing in techniques for manual treatment, injury recovery, pain reduction and reduction of fall risks.
- Wellness Coaching and post rehabilitation maintenance session ( 1 hour); focus on personal wellness goals or post rehab maintenance
- Home Safety Assessment (1 hour); to help promote increased safety and ease of daily tasks.
- Independent Contracting on an as needed basis with Rehabilitation Providers

(the above sessions are face to face time and do not include the clinician’s drive time to your home or preferred location which is included in the price of service)
(Appointment cancellation policy: contact us to cancel an appointment with at least 24 hour notice. Any missed appointments without cancellation notification will be charged a small fee as your appointment time is reserved solely for you. Extenuating circumstances and special situations will be reviewed on an individual basis.)
Physician Referral
- Physical Therapy treatment – a physician prescription or referral is not required for treatment within the first 30 days. However, it is recommended to have one if you have experienced a significant injury, illness or surgery and are currently under a physician’s care or if you plan to seek insurance reimbursement.
- Wellness Coaching and or Home Safety Assessments – a physician referral is not necessary.
Insurance Policy
Insurance is not accepted as we are considered out-of-network. However, receipts with customary billing codes and documentation are used and provided for physical therapy clients to submit to their insurance providers for reimbursement. The amount of reimbursement is completely dependent upon your insurance plan.
